Presenter Agreement Form
Thank for you for agreeing to present a lecture at the TOMA/TxACOFP 10th Annual Joint Convention which will be held June 7-11, 2017 at Renaissance Worthington Hotel, Fort Worth, Texas
HOTEL ARRANGEMENTS: You will receive (1) night hotel lodging of a standard room at the conference group rate. Any additional nights and food and room incidentals are the responsibility of the presenter. Presenters are responsible for making their own reservations. The group rate is $189 plus tax. Please use this link to book your hotel room. After you select your dates, click “Special Rates” then “Group Code” and enter TOMTOMA to receive our group rate. Room rate deadline is May 24, 2017.
Each session will have the following equipment: laptop, LCD projector package, laser pointer and wireless microphone. If your session requires additional audio visual equipment you must submit your request by email to firstname.lastname@example.org no later than May 22nd. Requests will be reviewed on a case by case basis. If you are a MAC user, we ask that you bring your personal MAC laptop with needed accessories.
TOMA is accredited by the American Osteopathic Association as a category 1A CME provider. For program accreditation, we require pre-and post-test questions be included in all presentation slides and all presentations must be evidenced-based and free of commercial bias.

*PRE & POST TEST QUESTIONS measure the knowledge gained from the presentation. An Audience Response System will be used by participants during the presentation. Please choose 3-5 questions to begin and end with. Test questions should be slides within your Power Point Presentation.
All presentations must be evidence-based and completely free of commercial bias (no logos, brand products etc...).
Your presentation will be reviewed by the planning committee and if it is determined to be commercially biased or include logos etc. it will be sent back to you for revision.
For handout purposes, all presentations, unless otherwise specified, will be posted to the conference website, accessible as a “read only file” to registered attendees. They will be deleted 90 day’s post-conference. There will not be paper handouts provided to attendees; if you wish to provide them you must prepare them and bring them to your session.
